Centura Health at Home brings a continuum of expert, innovative, compassionate care to people of all ages, in all the places they call home. Our more than 1,200 employees serve communities across the state, caring for more people in more places than any other organization of its kind in Colorado.  The Centura Health at Home continuum of care includes home care, telehealth services, rehabilitation therapies, palliative care, both in-patient and home hospice services and seven senior living communities offering independent living, assisted living, nursing homes, memory care and adult day programs.
